• Must have a Bachelor’s Degree or Masters degree in Social Work from an accredited university.
• Must have at least 1 -2 years of relevant post-graduate working experience
• Must be able to provide proof of certification and/or registration as a Social Worker in their province or state

Job Description
Principal Accountabilities
1. To undertake initial screening and establish the client's eligibility for support under the 1986 Disabled Persons Act.
2. To undertake completion of the Transition Pathway Plan which identifies the needs and aspirations of the young person and takes account of social care, health and education issues and guidance in conjunction with the young person and/or their carers.
3. To provide opinions to Education as to those young people with statements of special educational needs who are permanently and substantially disabled.
4. To support the maintenance of a database of all children and young people with statements of special educational needs who are substantially and permanently disabled.
5. To carry out, where appropriate, complex single or multidisciplinary assessments to meet the identified needs of the young person in conjunction with them, their carers and relevant professionals.
6. Together with the young person and/or their carers, to produce a care plan
7. Where required to use the existing market place in order to obtain the most appropriate and cost effective supplier of services consistent with the needs of the client.
8. To ensure that the agreed care plan can be provided within the available budget.
9. Where appropriate to negotiate one-off contracts with prospective service providers. This will require liaison with the Service Development Officers to ensure that such contracts and specifications are appropriate.
10. To monitor the care being provided to ensure that needs are continuing to be met.
11. To re-assess the needs of a young person periodically and, when appropriate, to amend the care plan following consultations with them and/or their carers.
12. To provide on-going support and advice to young people and their carers.
13. To identify areas of need that cannot be met and to systematically record and report the unmet need to the Team Manager.
14. To bring to the notice of the Team Manager any instances of perceived failure of a provider to deliver the agreed level of service.
15. To maintain all necessary records and documentation in line with council administration procedures and use computerised information systems.
16. May be responsible for the supervision of a member of staff within the team.
17. To assist with the production of standards of practice and training and development activities.
18. To take part in staff meetings and contribute to the development of the team.
19. To facilitate small groups involving young people and carers.
20. To liase with other professionals in partnership organisations, and including voluntary organisations.
21. To take part in working groups, sub-groups, etc, both within and outside the council.
22. To assist with the production of quality standards and performance measures and to ensure that they are adhered to.
23. To undertake such other appropriate responsibilities as may be delegated from time to time by the Team Manager.
