• Must have a Bachelor’s Degree or Masters degree in Social Work from an accredited university.
• Must have at least 1 -2 years of relevant post-graduate working experience
• Must be able to provide proof of certification and/or registration as a Social Worker in their province or state
• Good communication skills


Job Description

SPECIFIC DUTIES

Operational
Assessment and Planning

• Work in partnership with other professionals and agencies to undertake a holistic assessment of vulnerable adults and their families and carers within the legislative framework and using the eligibility criteria for services.

• Develop care plans, which will be informed by an assessment of need and risk and identify desired outcomes within clearly defined timescales.

• In developing care plans consider least restrictive options that encourage the service user’s independence and choice and control over their lives.

• Ensure that service users’ views, and/or families/carers views are fully recorded during the assessment process and wherever possible are reflected in the subsequent case planning.

• Negotiate with service users/families/carers to enable needs to be positively met within available resources.

• Prepare and present reports in a clear and logical way, and contribute to multi-agency/multi disciplinary conferences and meetings as appropriate.

• Ensure that follow up action from the conferences/meetings is undertaken.

• Include cost/’best value considerations in identification of resources and case planning.

• To undertake a comprehensive review of the assessment.

• To undertake assessments as a member of the multi-disciplinary team and the duties of care co-ordinator under CPA.

Commissioning Services

• Commission services for clients within best value guidance and following FACS/CCA assessment process.
Working with Service Users/Families/Carers

• Use a range of interventions, as agreed with the designated line manager/supervisor, to engage the service user/family/carer in working towards the objectives of the Care Plan. The range includes one to one work, group work, supporting self-help groups, enabling access to community resources and provision of advice as appropriate

• Working pro-actively with service users/families/carers to enable complex needs to be positively met within available resources.

Care Planning/Review

• Monitor and review care plans to ensure they meet assessed needs and service standards within statutory timescales. Take appropriate action where gaps are identified including and advising designated line manager/supervisor and other departments in line with departmental procedures.

• Ensure that the service user/families/carers are fully consulted as part of reviews or placement meetings.
Case Management

• Manage an assigned caseload in compliance with agreed casework objectives, statutory and departmental policy and procedural requirements.

• Advise the designated line manager/supervisor of proposed care plans, progress and achievements and seek authorisation for decision-making in line with departmental delegated powers and supervisory policy.

• Listen to the views of the service user/family/carers.

• Maintain accurate and up to date case records using departmental recording systems, which includes IT systems and that this complies with departmental policy and procedures.

• Ensure the above is carried out promptly so that information can be given to the service user, carer or external agencies and for performance monitoring purposes.

• Co-operate in providing statistical and other information to assess the effectiveness of social care services and to help the department achieve strategic objectives.

• Contribute to service reviews, projects and other activities, providing information for the planning and development of high quality cost effective services that are responsive to the changing needs of the target population.

• Inform Team Manager of service delivery issues that may have implications for the department and/or are likely to be subject to publicity whether positive or negative, ensuring that any concerns about Children in Need and Vulnerable Adults are identified immediately.

• Provide information as requested to assist with complaint investigations.

• Monitor the mental state of the individual clients, and revise the care plan

• Undertake ‘duty’ responsibilities under the direction of the duty senior or supervisor, seeking advice and guidance as appropriate.

• Prepare and present reports for Case Conferences, Panels, Tribunals and review meetings.

• Attend Court proceedings and Tribunals as required.

• Prepare court reports.

• To act as an Appropriate Adult within agreed guidelines.


• To refer to and present cases at the MAPPA, in accordance with the local guidelines and provide up to date information to the MAPPA.
Career Development

• Be required to attend for regular supervision meetings to reflect on:

i. Casework, including overall plans and objectives
ii. Career Development
iii. Accountability and priority of work

• Attend staff meetings.

• Prepare for and contribute to the annual appraisal and mid year review.

• Undertake identified training as necessary.

• Keep up to date with service and with social care developments.

• To undertake ASW training and undertake ASW duties.

• Attend training as required and maintain up to date professional knowledge in line with GSCC requirements.
General

• Be ‘Information Technology’ literate and willing to develop further skills in this area.

• Contribute to publicising services to people in the local community in the work you do.

• Carry out duties and responsibilities in accordance with the Council’s Core Values, EMAS, Equal Opportunities and Diversity policy, Health and Safety policy and all other borough and Group policies and joint procedures as agreed between the Mental Health Trust and the Council, and where relevant Mental Health Trust policies.

• Willing to work flexible as required, which may require the attendance at evening meetings.

• Undertake duties consistent with responsibilities of the post as required by the designated line manager/supervisor.

• Represent the department as required at internal, multi agency and public meetings.
